统一信封 structuredContent+schema_version+request_id（请求头 X-Mcp-Schema-Version: 2 双跑，默认老结构）；8 个统一错误码收敛；tools/list 注入只读/幂等/非破坏 annotations；日志手机号脱敏。单测 125+ 通过，test-up.sh 冒烟通过。
